Related services and offerings with links from this website, including vendor sites, have their own privacy statements that can be viewed by clicking on the corresponding links within each respective website. However, since we do not have direct control over the policies or practices of participating merchants and other third parties, we are not responsible for the privacy practices or contents of those sites. 3.1 WEB SERVER LOGS


When you visit our Website, we may track information to administer the site and analyze its usage. Examples of information we may track include: 



  • Your Internet protocol address. 

  • The kind of browser or computer you use. 

  • Number of links you click within the site. 

  • State or country from which you accessed the site. 

  • Date and time of your visit. 

  • Name of your Internet service provider. 

  • Web page you linked to our site from. 

  • Pages you viewed on the site.

3.2 USE OF WEB BEACONS OR GIF FILES


Feel Good Fuel Web pages may contain electronic images known as Web beacons – sometimes also called single-pixel gifs – that allow Feel Good Fuel to count users who have visited those pages and to deliver co-branded services. Feel Good Fuel may include Web beacons in promotional e-mail messages or newsletters in order to determine whether messages have been opened and acted upon. Some of these Web beacons may be placed by third party service providers to help determine the effectiveness of our advertising campaigns or email communications. These Web beacons may be used by these service providers to place a persistent cookie on your computer. This allows the service provider to recognize your computer each time you visit certain pages or emails and compile anonymous information in relation to those page views, which in turn enables us and our service providers to learn which advertisements and emails bring you to our website and how you use the site. When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.

An anonymized string created from your email address (also called a hash) may be provided to the Gravatar service to see if you are using it. The Gravatar service privacy policy is available here: https://automattic.com/privacy/. After approval of your comment, your profile picture is visible to the public in the context of your comment.

3.4 MEDIA

If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.

3.5 COOKIES

If you leave a comment on our site you may opt-in to saving your name, email address and website in cookies. These are for your convenience so that you do not have to fill in your details again when you leave another comment. These cookies will last for one year.

If you visit our login page, we will set a temporary cookie to determine if your browser accepts cookies. This cookie contains no personal data and is discarded when you close your browser.

When you log in, we will also set up several cookies to save your login information and your screen display choices. Login cookies last for two days, and screen options cookies last for a year. If you select “Remember Me”, your login will persist for two weeks. If you log out of your account, the login cookies will be removed.

If you edit or publish an article, an additional cookie will be saved in your browser. This cookie includes no personal data and simply indicates the post ID of the article you just edited. It expires after 1 day.

3.6 EMBEDDED CONTENT FROM OTHER WEBSITES

Articles on this site may include embedded content (e.g. videos, images, articles, etc.). Embedded content from other websites behaves in the exact same way as if the visitor has visited the other website.

These websites may collect data about you, use cookies, embed additional third-party tracking, and monitor your interaction with that embedded content, including tracking your interaction with the embedded content if you have an account and are logged in to that website.

3.7 CONTACT FORMS

Feel Good Fuel utilizes contact forms to gather user provided information. This may include but is not limited to Name, Mailing Address, Email, Cell Phone, user preferences and regulatory acknowledgements. This information will only be used for providing requested services, providing information about available services, product updates and other service related communications via multiple communication medias in which you have provided consent to receive communications.

3.8 ANALYTICS

Feel Good Fuel may track user information to administer the site, analyze its usage and performance, develop marketing and make service improvements. Your continued use of the services and website constitutes your agreement to gather information for analytics.

 

4. WHO WE SHARE YOUR DATA WITH

Feel Good Fuel utilizes merchants and third-party service providers to support execution of services. As such, only necessary information to facilitate participation in providing requested services will be shared with these merchants and third-parties. Under no circumstance will Feel Good Fuel provide any customer information to any other party not explicitly involved in the execution of the customer requested service. By utilizing Feel Good Fuel services you are in agreement and provide full consent of sharing of information to only merchants and third-party service providers involved in the execution of requested services. 

 

5. HOW LONG WE RETAIN YOUR DATA

If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.

For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.

 

6. WHAT RIGHTS YOU HAVE OVER YOUR DATA

If you have an account on this site, or have left comments, you can request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.

7. REGISTRATION AND PASSWORDS

The Online Services may permit or require you to register or obtain a password prior to permitting you to access certain portions of the Online Services.  You acknowledge and agree that you are responsible for maintaining the confidentiality of your login ID and password, and for all uses of your login ID, password, and/or Account.  You agree to notify us immediately of any unauthorized use of your login ID, password, or Account, or any other breach of security involving access to the Online Services through your Account. We may also provide Personal Information to a third party in connection with the sale, assignment, or other transfer of the business of this Website to which the information relates, in which case We will require any such third party to agree to treat Personal Information in accordance with Our Terms of Use.

We also may share aggregate, non-personal information about Website usage with unaffiliated third parties. This aggregate information does not contain any personal identifiable information about our users.

9. HIPAA INFORMATION

The Health Insurance Portability and Accountability Act of 1996 (HIPAA), requires the maintaining of privacy for health information that identifies you, called protected health information (PHI), and to provide you with notice of an organization’s legal duties and privacy practices regarding PHI. Feel Good Fuel does not maintain PHI and as such, has no legal duties or privacy practices under the HIPAA.

9.1 THIRD PARTY PARTNER’S PROTECTION OF PROTECTED HEALTH INFORMATION (PHI)

Our third party laboratory is a Clinical Laboratory Improvement Amendments of 1988 (CLIA) – certified facility. In accordance with CLIA and the HIPAA Privacy Rule, CLIA-certified labs may provide the patient, the patient’s personal representative, or a person designated by the patient, as applicable, with copies of completed test reports that, using the laboratory’s authentication process, can be identified as belonging to that patient.
